This notice explains how Billingham and Norton Primary Care Network (PCN) uses and protects patient information.
Who we are
Billingham and Norton Primary Care Network (PCN) is a group of GP practices working together to improve health services for our local population.
Each GP practice remains an independent Data Controller for patient medical records.
Why we use patient information
We use patient information to:
- Provide direct patient care
- Plan and improve health services
- Support population health management
- Meet NHS and legal obligations
What information we use
This may include:
- Name, address, date of birth
- NHS number
- Contact details
- Relevant health and care information
Lawful basis for processing
We process patient data under:
- Article 6(1)(e) – task carried out in the public interest
- Article 9(2)(h) – management of health or social care systems
Who we share information with
We may share information with:
- GP practices within the PCN
- NHS Integrated Care Board (ICB)
- NHS England
- Community, mental health and hospital services
- Approved data processors under contract
Information is shared only when necessary and lawful.
How long we keep records
Patient records are retained in line with:
- NHS Records Management Code of Practice
Your rights
You have the right to:
- Access your medical records
- Request corrections
- Object to certain uses of your data
- Raise concerns about how your data is used
Requests should usually be made to your GP practice.
